An exciting opportunity is available for a Registered Nurse specializing in Case Management, focusing on discharge planning and patient-centered care in a dynamic acute care hospital environment. This contract role is ideal for professionals with a proven background in complex case management within large organizations and teaching hospitals.

Schedule:

  • 40 hours per week
  • Day shift, Monday to Friday
  • Schedule may be 4x10 hour days or 5x8 hour days, based on operational needs

Qualifications:

  • Valid RN license required
  • Minimum of 2 years of case management experience
  • At least 6 months of recent discharge planning experience in an acute care hospital setting
  • Experience working in large organizations or teaching hospitals with 500+ beds is highly preferred
  • Strong knowledge of care coordination, discharge planning, utilization review, and interdisciplinary team collaboration
  • Excellent communication, critical thinking, and organizational skills

Responsibilities:

  • Collaborate closely with interdisciplinary teams to ensure optimal patient outcomes throughout the continuum of care
  • Develop individualized care transition plans and support safe, effective discharge processes
  • Conduct assessments and provide education for patients and families about post-acute care options and available resources
  • Monitor patient progress, evaluate care plans, and modify as needed to align with best practices
  • Advocate for patients and facilitate connections to community, social, and clinical support services
