Seeking a highly skilled Registered Nurse with expertise in Case Management to support an acute care hospital in Cambridge, MA. The successful candidate will excel in collaborative discharge planning, patient advocacy, and care coordination within a dynamic, large-scale hospital setting. This assignment is based on a contract and offers a day shift schedule with 40 hours per week—scheduling to be determined by operational needs (either four 10-hour shifts or five 8-hour shifts, Monday through Friday).
Schedule/Hours:
- 40 hours per week
- Day shift, Monday through Friday
- Schedule may be 4x10-hour shifts or 5x8-hour shifts, based on department need
Qualifications:
- Valid RN license and good standing
- Minimum 2 years of recent RN Case Management experience required
- At least 6 months of discharge planning experience in Case Management within an acute care hospital setting
- Experience in large hospitals (500 beds or more), preferably within a teaching environment
- Excellent communication, organizational, and critical thinking skills
Responsibilities:
- Conduct patient assessments for safe and effective discharge planning
- Collaborate with interdisciplinary teams to coordinate inpatient care and transitions
- Advocate for patients and facilitate connections to community and post-acute resources
- Ensure compliance with regulatory standards and hospital policies
- Document case management activities promptly and accurately
- Support patients and families through complex care situations and discharge processes
