Advance your nursing expertise in a dynamic, high-acuity environment providing acute inpatient case management and discharge planning services. This contract opportunity offers an engaging role within a multidisciplinary team in a large, fast-paced Level 1 trauma and academic medical center, where no two days are the same.
Schedule/Hours
- Monday through Friday, 8:00 a.m. to 4:30 p.m.
- No weekend shifts
- Holiday coverage as needed, rotated among staff
Required Qualifications & Experience
- Active RN license and compliance with all facility requirements, including completion of background and fingerprinting (remote or in-person options available)
- Minimum 2 years’ recent acute inpatient medical case management experience, with at least 1 year (within the past 3 years) in a major busy hospital emergency department (500+ beds, Level 1/academic medical center)
- Documented acute care discharge planning (within last 2 years) across diverse medical diagnoses
- Previous travel experience in a Level 1 facility within the past 3 years
- Proficient in EPIC electronic health records
- Strong clinical assessment, organizational, and critical thinking skills
- Experience must specifically center on acute inpatient settings; telephonic, utilization review, insurance, rehab, LTC, or hospice-only backgrounds do not meet requirements
- Flu vaccination mandatory (only documented medical exemptions considered)
Core Responsibilities
- Manage care coordination and discharge planning for 15–20 patients daily across all inpatient units
- Collaborate closely with an extensive Social Work team and consult with Respiratory Therapy and Pharmacy as needed for complex placements
- Ensure efficient transitions of care for a medically diverse patient population, including those with limited financial resources
- Adapt rapidly to minimal training and maintain team collaboration—never working alone
- Uphold facility protocols and contribute to high-quality patient outcomes

$2,105 - $2,227/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
