Schedule
- 5x8-hour shifts, 40 hours per week guaranteed
- Rotating 36/48 schedule: 7a-7p every Wednesday & Thursday, plus every other Friday, Saturday, and Sunday
Key Qualifications
- Active LMSW or LCSW and MS State License
- BLS certification and Compact State License for nurses
- At least 2 years of acute inpatient medical case management experience, including 1 year in the past 3 within a large (500+ beds) Level 1 or academic hospital
- Prior travel experience in a Level 1 facility (within 3 years)
- Demonstrated proficiency with EPIC charting and strong computer skills
- Acute care/ED discharge planning experience within the last two years
- Minimal unit training—must work efficiently and learn quickly
- Flu vaccine required (medical exemptions only with documentation)
- Ability to meet fingerprinting process requirements
Role Responsibilities
- Manage caseloads of 15-20 patients daily with complex, acute care placement needs
- Collaborate with RT, Pharmacy, and the interdisciplinary care team for discharge planning
- Utilize EPIC for detailed documentation and care coordination
- Navigate patient placement options among multiple contracted facilities
- Provide comprehensive discharge planning for a variety of trauma and complex cases
- Adhere to all regulatory, documentation, and compliance standards
