Make a meaningful impact as part of a multidisciplinary care team as you deliver comprehensive case management services within an acute inpatient setting. This contract position demands strong clinical expertise and a compassionate, patient-centered approach to drive optimal health outcomes across diverse patient populations.

Schedule:

  • Five 8-hour day shifts per week
  • Every other weekend coverage required

Key Qualifications:

  • Active RN license required
  • Minimum of 2 years’ recent acute inpatient case management experience (mandatory)
  • Strong clinical assessment and care coordination skills
  • Expertise in motivational interviewing and supporting patients through complex care transitions
  • Ability to collaborate with multidisciplinary teams and engage patients and families as partners in care
  • Proficiency in care planning, resource identification, and advocacy
  • Outstanding interpersonal, written, and verbal communication skills

Primary Responsibilities:

  • Perform comprehensive assessments of patient and family needs (biomedical, psychological, social, and functional)
  • Create and implement individualized, patient-centered care plans focused on safety, quality, and cost-effectiveness
  • Engage patients and families through ongoing communication, education, and advocacy
  • Address complex management needs for critical events or diagnoses, ensuring coordination and safe care transitions
  • Provide counseling on treatment options and advanced care planning, including end-of-life decision support
  • Collaborate with physicians, specialists, and social workers to facilitate seamless care
  • Support process improvement initiatives to reduce risk and enhance patient outcomes
  • Intervene and facilitate support in cases involving abuse and sensitive family dynamics
