Deliver impactful care as a Registered Nurse specializing in Case Management in a dynamic, patient-focused setting. This contract opportunity involves serving as an integral member of a patient-centered care team, coordinating comprehensive health services, and ensuring effective transitions across the continuum of care. The role places a strong emphasis on assessment, care planning, facilitation, advocacy, and resource management for patients requiring complex case management due to critical events or challenging diagnoses.

Schedule:

  • 5 shifts per week, 8-hour days
  • Every other weekend required

Qualifications:

  • Current RN license in good standing
  • Minimum of 2 years’ recent acute inpatient case management experience
  • Strong clinical assessment, critical thinking, and motivational interviewing skills
  • Ability to handle multiple priorities in a fast-paced healthcare environment
  • Excellent communication, collaboration, and advocacy abilities

Responsibilities:

  • Conduct thorough assessments addressing patients’ biomedical, psychological, social, and functional needs
  • Develop and implement patient-centered care plans to optimize outcomes and the care experience
  • Engage patients and families in the care process, providing education, resources, and advocacy
  • Guide and support patients in treatment decisions and advanced care planning
  • Ensure safe and seamless transitions between care settings
  • Coordinate interventions related to abuse or neglect when needed
  • Participate in continuous improvement initiatives and analyze outcomes to enhance quality of care
  • Collaborate extensively with interdisciplinary team members to co-manage complex medical and psychosocial cases
  • Maintain high standards in documentation, compliance, and process improvements
