Make a meaningful impact as part of a multidisciplinary care team as you deliver comprehensive case management services within an acute inpatient setting. This contract position demands strong clinical expertise and a compassionate, patient-centered approach to drive optimal health outcomes across diverse patient populations.
Schedule:
- Five 8-hour day shifts per week
- Every other weekend coverage required
Key Qualifications:
- Active RN license required
- Minimum of 2 years’ recent acute inpatient case management experience (mandatory)
- Strong clinical assessment and care coordination skills
- Expertise in motivational interviewing and supporting patients through complex care transitions
- Ability to collaborate with multidisciplinary teams and engage patients and families as partners in care
- Proficiency in care planning, resource identification, and advocacy
- Outstanding interpersonal, written, and verbal communication skills
Primary Responsibilities:
- Perform comprehensive assessments of patient and family needs (biomedical, psychological, social, and functional)
- Create and implement individualized, patient-centered care plans focused on safety, quality, and cost-effectiveness
- Engage patients and families through ongoing communication, education, and advocacy
- Address complex management needs for critical events or diagnoses, ensuring coordination and safe care transitions
- Provide counseling on treatment options and advanced care planning, including end-of-life decision support
- Collaborate with physicians, specialists, and social workers to facilitate seamless care
- Support process improvement initiatives to reduce risk and enhance patient outcomes
- Intervene and facilitate support in cases involving abuse and sensitive family dynamics
