Registered Nurse Case Manager (RN) - Detroit, Michigan

We are currently seeking experienced Registered Nurse Case Managers to provide integrated case management for patients with complex needs. This contract opportunity emphasizes care coordination, patient advocacy, and a holistic approach to health outcomes along the care continuum.

Schedule:

• 5 shifts per week, 8-hour days
• Every other weekend required

Qualifications:

• Active RN license in good standing
• Minimum of two years’ recent acute inpatient case management experience required
• Demonstrated ability in assessment, planning, facilitation, and care coordination for diverse populations
• Strong skills in motivational interviewing, patient education, and resource identification
• Experience in collaborating with multidisciplinary teams to achieve optimal patient outcomes
• Exceptional critical thinking, communication, and documentation abilities

Responsibilities:

• Conduct comprehensive assessments of patients’ biomedical, psychological, social, and functional needs
• Develop individualized care plans focused on quality, cost-effective outcomes and a superior patient experience
• Engage and support patients and families through effective communication, advocacy, and health education
• Apply judgment and motivational techniques to help patients overcome barriers and reach goals
• Offer counseling and interventions for treatment decisions, end-of-life issues, and advanced care planning
• Coordinate safe transitions of care across inpatient, outpatient, and health plan settings
• Advocate for appropriate services considering the patient’s benefit structure
• Facilitate multidisciplinary care conferences and collaborate in managing complex cases
• Continually evaluate and adjust care plans per patient response and evolving needs
• Participate in process improvement projects, risk reduction, and intervention strategies
• Intervene in sensitive cases such as abuse, neglect, or violence as necessary
