Advance your nursing career by joining a dynamic inpatient case management team at a major academic medical center. This contract position requires a highly skilled RN committed to excellence in acute patient management and complex discharge planning. The hospital environment is fast-paced and serves a diverse population, including a significant volume of trauma and patients with challenging placements.
Schedule:
- Monday to Friday, 8:00 am – 4:30 pm
- No weekend requirements
- Holiday coverage rotated as needed
Required Qualifications:
- Minimum of 2 years’ acute inpatient medical case management experience
- At least 1 year (in the last 3) in a large, busy hospital ED (500+ beds, Level 1/academic medical center)
- Acute care discharge planning experience within the last 2 years for a wide range of diagnoses
- Prior travel assignment in a Level 1 trauma facility within the last 3 years
- Strong EPIC charting proficiency and advanced computer skills
- Experience in telephonic/utilization management, insurance, rehab, LTC, or hospice will not be considered
- Ability to work independently with minimal training; must adapt quickly
Responsibilities:
- Manage a caseload of 15-20 patients daily, covering all inpatient units
- Coordinate individualized discharge plans for patients with diverse medical needs
- Collaborate closely with a robust social work team and consult RT or pharmacy for discharge as needed
- Navigate complex placements and financial assistance criteria, supporting uninsured and underinsured populations
- Document and report using EPIC
- Attend a two-week orientation and ongoing support from the case management team

- 50-mile radius rule applies for travelers
- Flu vaccine required (medical exemption considered with provider documentation)
- Fingerprinting required (process can be completed remotely or onsite)
- Scrubs in any color/business professional attire accepted

$2,105 - $2,227/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
