Schedule:
- 40 hours per week (dependent on operational need, may be four 10-hour shifts or five 8-hour shifts)
- Day shift, Monday through Friday
Qualifications:
- Current RN license
- Minimum of 2 years of case management experience
- At least 6 months of discharge planning case management experience within an acute care hospital setting
- Prior work in a large organization or teaching hospital with 500+ beds highly preferred
- Strong clinical assessment, problem-solving, and communication skills
- Ability to navigate interdisciplinary relationships and advocate for patients
Key Responsibilities:
- Assess patients for appropriate post-acute level of care and develop discharge plans tailored to individual needs
- Collaborate with physicians, social workers, and other healthcare team members on complex care plans
- Coordinate community resources, follow-up appointments, and ongoing care arrangements
- Maintain thorough, compliant documentation in electronic health records
- Communicate effectively with patients and families to educate them about care transitions and available resources
- Monitor and evaluate outcomes, making adjustments to plans as necessary for optimal patient care

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
