Schedule & Hours

  • 5x8 hour shifts, 40 hours per week guaranteed
  • Rotating 36/48 schedule: 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday
  • Department orientation provided (2 weeks)
  • No weekend requirements

Qualifications & Required Experience

  • Active LMSW or LCSW and active MS state license
  • Minimum 2 years of acute inpatient medical case management (with at least 1 year in a 500+ bed, Level 1/Academic Medical Center within the last 3 years)
  • Experience with discharge planning in acute care or emergency department settings
  • Proficiency with EPIC charting
  • Prior travel experience in a Level 1 facility within the past 3 years
  • BLS certification and compact state license
  • Recent relevant experience essential; able to adapt quickly and function independently with minimal training
  • Strong computer and communication skills

Key Responsibilities

  • Manage a caseload of 15-20 complex inpatients
  • Provide comprehensive discharge planning and facilitate patient transitions
  • Consult with respiratory therapy and pharmacy as needed for care planning
  • Ensure compliance with facility protocols, including all vaccination and fingerprinting requirements
