Desired Qualifications:

• Current, active RN license in good standing

Key Responsibilities:

• Assess, plan, implement, coordinate, and evaluate options and services for assigned patients

• Serve as a patient advocate, ensuring continuity of care and appropriate resource utilization
• Facilitate and document communications between healthcare professionals, patients, and external agencies
• Educate patients and family members on care plans, post-discharge needs, and available resources
