Make a direct impact in acute inpatient case management, supporting medically complex patients in a dynamic, high-volume environment. This contract position is designed for an RN with proven expertise in Level 1 trauma centers, specializing in patient assessment, discharge planning, and collaborative care. Your ability to work efficiently with minimal training will ensure seamless transitions for patients throughout the hospital.
Schedule/Hours:
- Monday through Friday, 8:00am – 4:30pm
- No weekend shifts
- Rotating holiday coverage as needed
Qualifications:
- Active RN license and current flu vaccination (medical exemptions require documentation)
- Minimum 2 years of recent acute inpatient medical case management experience, including at least 1 year (within the last 3 years) in a 500+ bed Level 1/Academic ED
- Strong, recent (within last 2 years) acute care discharge planning experience with a diverse patient population
- Previous travel assignment in a Level 1 facility within the past 3 years
- Proficient in EPIC charting and strong computer skills
- Experience with utilization management, telephonic, insurance company, rehab, LTC, or hospice is not applicable to this role
Responsibilities:
- Manage a caseload of 15-20 patients daily, covering all inpatient units
- Collaborate with the Social Work team to develop and implement effective discharge plans
- Coordinate with respiratory therapy and pharmacy as needed for discharge planning
- Support patients with complex placement needs and assist in navigating financial assistance criteria
- Participate in a two-week orientation and ongoing teamwork

$2,105 - $2,227/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
