Registered Nurse Case Manager in Quincy, MA

Play a key role in patient care coordination and support as a Registered Nurse specializing in Case Management within an acute care setting. This contract opportunity involves collaborating with multidisciplinary teams to ensure effective and efficient discharge processes for patients in a dynamic hospital environment.

Schedule/Hours:

• 40 hours per week (may be scheduled as 4x10's or 5x8's, depending on operational needs)
• Day shift, Monday through Friday

Desired Qualifications and Experience:

• Active RN license
• Minimum of 2 years of nursing experience in a case management role
• At least 6 months of recent discharge planning case management experience in an acute care hospital (ideally a large teaching organization with 500+ beds)
• Strong communication, organizational, and assessment skills
• Demonstrated ability to collaborate with interdisciplinary teams and advocate for patient needs
• Familiarity with care planning, utilization review, and resource management

Key Responsibilities:

• Coordinate discharge planning and care transitions for patients throughout their hospital stay
• Conduct comprehensive case assessments to determine patient needs and appropriate resources
• Collaborate with physicians, specialists, and ancillary staff to develop individualized care plans
• Communicate regularly with patients, families, and community resources to support care continuity
• Ensure timely and safe discharge processes, addressing barriers and facilitating solutions
• Maintain accurate case management documentation and comply with all regulatory requirements
