Step into a critical care coordination role, delivering high-impact case management services supporting patients across the care continuum. This opportunity emphasizes collaboration, patient advocacy, and the delivery of quality, cost-effective care in a dynamic healthcare environment.
Schedule/Hours:
- Five 8-hour shifts per week
- Day shifts
- Every other weekend required
Preferred Qualifications and Experience:
- Active RN license
- Minimum of 2 years recent acute inpatient case management experience
- Proficiency in assessment, planning, and facilitation of care
- Demonstrated ability to engage with diverse patient populations
- Strong communication skills for ongoing patient, family, and team interaction
- Acumen in motivational interviewing, self-management support, and clinical counseling
- Background in handling complex management strategies for patients with significant medical or psychosocial needs
Key Responsibilities:
- Conduct comprehensive assessments to determine patients’ biomedical, psychological, social, and functional care needs
- Design and implement patient-centered care plans tailored for optimal outcomes and safety
- Facilitate communication and seamless transitions between acute and ambulatory care settings
- Champion patient advocacy in care decisions, advanced care planning, resource identification, and delivery of services
- Employ clinical judgment and critical thinking to support patients through barriers and complex transitions
- Lead and participate in interdisciplinary care team conferences, especially for cases with multidimensional needs
- Evaluate patient progress, updating care plans as required for continued improvement
- Participate in quality improvement initiatives, including risk reduction, data analysis, and follow-up actions
- Address and intervene in cases involving abuse, neglect, or other vulnerable adult and child protection issues
