Registered Nurse Case Manager in Rochester, MI

Schedule/Hours:

• Five 8-hour shifts per week
• Day shifts
• Every other weekend required

Preferred Qualifications and Experience:

• Active RN license
• Minimum of 2 years recent acute inpatient case management experience
• Proficiency in assessment, planning, and facilitation of care
• Demonstrated ability to engage with diverse patient populations
• Strong communication skills for ongoing patient, family, and team interaction
• Acumen in motivational interviewing, self-management support, and clinical counseling
• Background in handling complex management strategies for patients with significant medical or psychosocial needs

Key Responsibilities:

• Conduct comprehensive assessments to determine patients’ biomedical, psychological, social, and functional care needs
• Design and implement patient-centered care plans tailored for optimal outcomes and safety
• Facilitate communication and seamless transitions between acute and ambulatory care settings
• Champion patient advocacy in care decisions, advanced care planning, resource identification, and delivery of services
• Employ clinical judgment and critical thinking to support patients through barriers and complex transitions
• Lead and participate in interdisciplinary care team conferences, especially for cases with multidimensional needs
• Evaluate patient progress, updating care plans as required for continued improvement
• Participate in quality improvement initiatives, including risk reduction, data analysis, and follow-up actions
• Address and intervene in cases involving abuse, neglect, or other vulnerable adult and child protection issues
