Key Qualifications:

• Current RN license, in good standing
• Prior experience in acute care Case Management or Utilization Review preferred
• Strong assessment, communication, and critical thinking skills
• Proficiency in managing transitions of care, discharge planning, and collaborative interdisciplinary teamwork
• Ability to navigate electronic medical records and documentation efficiently

Responsibilities:

• Coordinate and monitor patient care plans
• Collaborate with multidisciplinary teams to ensure continuity and quality of care
• Advocate for patients and families, providing education on care options and resources
• Conduct ongoing assessment and evaluation of patient progress, adjusting plans as necessary
• Facilitate effective transitions from hospital to home or other care settings
