Key Qualifications:
- Current RN license, in good standing
- Prior experience in acute care Case Management or Utilization Review preferred
- Strong assessment, communication, and critical thinking skills
- Proficiency in managing transitions of care, discharge planning, and collaborative interdisciplinary teamwork
- Ability to navigate electronic medical records and documentation efficiently
Responsibilities:
- Coordinate and monitor patient care plans
- Collaborate with multidisciplinary teams to ensure continuity and quality of care
- Advocate for patients and families, providing education on care options and resources
- Conduct ongoing assessment and evaluation of patient progress, adjusting plans as necessary
