Registered Nurse Case Manager (RN) - Warren, Michigan

Enjoy a vital role supporting patients and families through comprehensive case management within an acute inpatient setting. This opportunity requires recent experience as an RN case manager in an acute care environment, ensuring high-quality patient transitions and optimized health outcomes.

Schedule:

• 5x8 hour day shifts
• Every other weekend required

Qualifications:

• Valid RN license required
• Minimum 2 years of recent acute inpatient case management experience
• Strong assessment, planning, and care coordination expertise
• Skilled in motivational interviewing and patient advocacy
• Proficient in communication, collaboration, and critical thinking

Responsibilities:

• Conduct comprehensive assessments of patients and family/caregiver needs across biomedical, psychological, social, and functional domains
• Develop and implement individualized, patient-centered care plans supporting optimal recovery and experience
• Engage patients and families as partners through education, advocacy, and ongoing communication
• Empower patients to overcome barriers using counseling and evidence-based intervention techniques
• Counsel patients and families on treatment choices and end-of-life issues, including advanced care planning
• Ensure seamless, safe transitions across care settings by coordinating services and resources
• Advocate for appropriate care delivery within health plan benefits
• Collaborate with interdisciplinary teams on complex medical and social cases, including the facilitation of collaborative care conferences
• Adjust care plans based on ongoing patient evaluation and outcomes
• Participate in quality improvement activities, including risk reduction interventions and data analysis
• Serve as a key responder for cases involving abuse or violence
• Support organizational goals through consistent, high-quality case management
