This 1:1 assignment is based in St. Charles, providing individualized care for a student with Down Syndrome. The primary responsibilities revolve around medical monitoring and support throughout the school day.

Key Qualifications:

• Active LPN/LVN license in Illinois (unrestricted)
• Experience with pediatric care, preferably in a school-related or similar environment
• Comfortable managing g-tube feedings
• Familiarity with diabetes care, including blood glucose monitoring, carb counting, and insulin administration
• Ability to use health-tracking applications such as Blue Loop or willingness to learn
• Demonstrated attention to detail, strong communication, and a compassionate, student-focused approach

Responsibilities:

• Oversee and provide about eight g-tube feedings during the school day
• Monitor and manage blood glucose levels; record and track readings regularly
• Administer insulin and ensure proper dosage in accordance with physician’s instructions
• Accurately count carbohydrates to support diabetes management
• Maintain detailed documentation of all medical interventions and observations
• Serve as a reliable, caring presence for the designated student, supporting their health and comfort in the classroom environment
• Collaborate with teachers, school health staff, and parents as needed
