School-Based Physical Therapist - Antioch, IL

Qualifications & Experience:

• Active Physical Therapist (PT) license and registration, valid for school-based practice in Illinois
• Previous experience delivering physical therapy in a school or pediatric setting preferred
• Strong collaboration and communication abilities with students, teachers, and parents
• Commitment to fostering a positive, supportive, and inclusive school culture
• Ability to adapt therapy plans to meet diverse student needs

Responsibilities:

• Conduct comprehensive assessments to determine students’ physical therapy needs within the educational setting
• Develop and implement individualized treatment plans and therapeutic activities
• Work closely with teachers, support staff, and families to support students’ mobility, access, and participation in school programs
• Participate in Individualized Education Program (IEP) team meetings and contribute to goal-setting and progress monitoring
• Maintain clear and accurate documentation, following district and federal guidelines
• Provide in-service training and support to staff as needed regarding safe movement and adaptive equipment
• Foster a nurturing environment that encourages student independence and confidence
