School-Based Physical Therapist - Chicago Heights, Illinois

An exciting contract opportunity is available for a skilled Pediatric Physical Therapist seeking experience in a school-based setting near Chicago Heights, IL. This role offers an engaging and diverse caseload spread across three buildings, working closely with students and multidisciplinary teams to support physical development and learning outcomes.

What We’re Looking For:

• Valid Physical Therapist license in Illinois
• Experience in school-based or pediatric settings preferred
• Familiarity with managing IEP-related services and documentation
• Strong communication and collaboration skills
• Organized, flexible, and comfortable working across multiple buildings
• Ability to work independently and as part of a team

Key Responsibilities:

• Provide direct and consultative PT services to students per their IEPs, adjusting interventions as needed
• Deliver individualized therapy sessions ranging from 0–60 minutes per week, depending on student needs
• Serve 5 students in Building 1 (sessions vary from 7.5–60 minutes weekly)
• Support 7 students in Building 2, with both direct and consultation models
• Collaborate with a cooperative program in Building 3 for 5–6 students; typically, 45–60 minute weekly sessions
• Maintain accurate, timely documentation and progress reporting for each student
• Participate in IEP meetings, collaborating with staff and families to ensure the best outcomes
• Travel between buildings as assigned over the 3–4 scheduled workdays
