School-Based Physical Therapist - Columbia City, Indiana

Qualifications and Experience:

• Valid state licensure or eligibility to practice as a Physical Therapist
• Experience providing physical therapy services in a school setting preferred

• Familiarity with IEP processes and documentation requirements

Key Responsibilities:

• Conduct evaluations and develop individualized treatment plans for students

• Maintain detailed documentation for compliance with state and federal regulations
• Participate in IEP meetings and provide expert recommendations on mobility and access
• Monitor student progress and adjust therapy plans as necessary
