School-Based Physical Therapist - Concord, CA

Make a meaningful difference in the 2026-2027 school year as a part-time Physical Therapist working with students in a supportive school environment. Desired Experience & Qualifications:

• California Physical Therapy license in good standing
• Experience providing PT services in school settings or with pediatric populations
• Strong understanding of IEP processes and documentation
• Collaborative and flexible attitude working with multidisciplinary teams
• Excellent communication and organizational skills

Core Responsibilities:

• Conduct assessments and evaluations to determine students’ physical therapy needs
• Develop, implement, and monitor individualized treatment plans
• Deliver direct therapy sessions to students within the school environment
• Maintain timely and accurate records, including progress notes and IEP documentation
• Consult with teachers, staff, and families to support optimal outcomes for students
• Adapt therapy activities to fit the school setting and student goals
• Participate in IEP meetings and professional development, as needed
