School-Based Physical Therapist - Des Plaines, Illinois

Qualifications:

• Valid Physical Therapist license for the state of Illinois
• Previous experience in school settings or with pediatric populations strongly preferred
• Familiarity with IEP development and progress monitoring
• Ability to communicate effectively with students, families, and interdisciplinary teams
• Strong organizational and time management skills to balance caseloads and documentation requirements

Responsibilities:

• Provide school-based physical therapy services for students with diverse needs
• Conduct evaluations, develop plans of care, and implement interventions to support student participation and independence
• Collaborate with educators, parents, and related service providers to integrate therapy goals into the classroom
• Maintain accurate and timely records, including progress reports and documentation for IEPs
• Support students in achieving milestones and maximizing their potential across the school day
