Physical Therapist - School Setting in Elmwood Park, IL

Key Qualifications:

• Licensed Physical Therapist with active credentials to practice in Illinois
• Prior experience in a school-based setting preferred
• Familiarity with both in-person and homebound service delivery
• Strong documentation, communication, and collaboration skills
• Ability to adapt interventions to a variety of educational environments

Primary Responsibilities:

• Provide direct PT services to a caseload of approximately 20 students, focusing on both in-person and select homebound learners
• Assess student needs and develop tailored intervention plans
• Work collaboratively with teachers, staff, and families to integrate therapy goals into the academic setting
• Maintain accurate records and progress notes
• Participate in IEP meetings and contribute to multidisciplinary teams
