This contract opportunity places you at the heart of an established outpatient program serving ages 12 through older adulthood—with a primary emphasis on young adults. You’ll play a key role in supporting functional independence and improving quality of life for a diverse population, all within a collaborative, year-round program.
Key Qualifications & Experience:
- Licensed Physical Therapist with experience in school-based or outpatient environments
- Comfortable working with individuals with a broad spectrum of intellectual and developmental disabilities
- Knowledgeable about the Medicare and Medicaid systems as they apply to patient care (no script writing required)
- Strong interpersonal skills, able to connect, motivate, and form meaningful relationships with patients
- Experience with adolescents and young adults preferred
- Open, adaptive communicator—able to thrive in a team and independently
Primary Responsibilities:
- Deliver individualized physical therapy to patients, typically 15-20 hours per week
- Develop tailored treatment plans focusing on improving mobility, daily living skills, and overall well-being
- See most patients twice per week in one-on-one sessions
- Collaborate closely with interdisciplinary team members and families
- Track progress, adjust goals, and maintain detailed patient records within regulatory frameworks
- Engage patients through thoughtful, positive interactions—personality and rapport are essential
