School-Based Physical Therapist - Evanston, Illinois

Qualifications:

• Licensed Physical Therapist with credentials to practice in Illinois
• Experience working with school-aged children or in an educational setting preferred
• Strong communication and collaboration skills with educators, families, and other professionals
• Ability to adapt treatment plans to meet the individual needs of students

Key Responsibilities:

• Provide direct physical therapy services to students, both one-on-one and in group settings
• Develop, implement, and track individualized treatment plans based on student needs and IEP/504 plan requirements
• Collaborate with teachers, staff, and parents to support classroom participation and student goals
• Maintain accurate and timely documentation in accordance with school and state guidelines
• Participate in IEP meetings, student evaluations, and interdisciplinary team discussions
• Assist in training school staff on strategies and accommodations as needed
