School-Based Physical Therapist - Middletown, New York

Key Qualifications:

• Valid Physical Therapist license or Doctor of Physical Therapy (DPT) degree
• Experience delivering PT services within a school setting, especially to students with mandated IEPs
• Comfortable conducting evaluations and reevaluations
• Ability to participate constructively in CSE meetings and work with multidisciplinary teams
• Strong organization and communication skills

Position Responsibilities:

• Provide direct PT interventions to elementary and middle school students, supporting their educational needs and promoting optimal physical function
• Complete thorough assessments and reevaluations in alignment with IEP requirements and timelines
• Attend CSE meetings and contribute insight regarding each student’s progress and recommended accommodations
• Maintain detailed documentation for compliance and progress tracking
• Collaborate with teaching staff and families to ensure holistic support for each student
