Embark on a highly impactful contract opportunity delivering Physical Therapy support within a dynamic year-round outpatient program tailored for individuals with intellectual and developmental disabilities. This unique setting offers the chance to foster meaningful progress and relationships while working with clients ranging from 12 years old to older adults, with a primary focus on young adults. Sessions typically occur twice weekly for each patient, allowing for consistent, collaborative care and the development of therapeutic rapport.

The program includes a broad spectrum of disabilities, but no restraints are required. As a provider, your knowledge of the Medicare and Medicaid system will be essential, although there is no expectation for script writing. Preferred Experience & Skills:

  • Current licensure as a Physical Therapist in New York State
  • Experience working in school, outpatient, or healthcare-based PT settings
  • Familiarity with supporting individuals with intellectual and developmental disabilities (I/DD)
  • Demonstrated success connecting with patients through strong communication and relationship-building
  • Understanding of Medicare/Medicaid system and requirements
  • Adaptability in collaborating with a multidisciplinary team (OT/PT professionals)
  • Professionalism, patience, and a genuinely caring demeanor

Key Responsibilities:

  • Deliver physical therapy services in an outpatient, year-round program
  • Provide consistent care to patients averaging 15–20 hours per week
  • Develop and implement individualized therapy plans
  • Maintain thorough documentation in compliance with regulatory requirements
  • Communicate effectively with patients, families, and team members
