School-Based Physical Therapist - San Juan Bautista, California

Qualifications:

• Active Physical Therapist license in California
• Previous experience in a school-based setting is highly desirable
• Strong assessment, documentation, and communication skills
• Comfortable working independently and traveling between schools
• Experience serving students from early childhood (preschool) through adolescence (high school)
• Ability to collaborate with educators, families, and other service providers

Responsibilities:

• Provide direct and consultative physical therapy services to students ages preschool through high school
• Conduct evaluations and re-evaluations to determine needs and progress
• Develop and implement individualized intervention plans based on student IEPs
• Maintain accurate, thorough records and timely documentation
• Participate in multidisciplinary team meetings, collaborating with teachers and related service team members
• Travel between assigned schools as part of an itinerant schedule
