Position Details

Responsibilities

• Provide case management services and ongoing support to assigned clients.
• Conduct assessments and develop individualized service plans.

• Maintain accurate and timely documentation.

• Participate in team meetings and collaborate with interdisciplinary staff as needed.

Qualifications

• Bachelor's or Master's degree in Social Work (BSW/MSW) or a related field.
• New Jersey Social Work license (LSW or LCSW) preferred, if applicable.
• Previous case management experience preferred.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently and effectively manage a flexible schedule.
