School-Based Social Worker - Evergreen, Colorado

Key Qualifications:

Primary Responsibilities:

• Provide targeted social-emotional and behavioral support to students
• Conduct assessments to identify barriers to learning and social development
• Partner with families, teachers, and administrators to create effective support plans
• Lead and participate in team meetings to review student progress
• Maintain accurate records and comply with reporting requirements
• Serve as a resource and advocate for students navigating social, emotional, and community challenges
• Assist in gathering and maintaining caseload details and relevant student information
