Deliver occupational therapy in patients’ homes, assisting individuals of all ages who experience cognitive or physical impairments due to injury, illness, or disability. Responsibilities:
- Provide occupational therapy assessments and treatments in patient residences.
- Collaborate with interdisciplinary teams to develop patient-centered care plans.
- Manage technical and environmental resources to support therapy delivery.
- Document patient progress using electronic health records and hand-held devices.
- Adapt interventions based on patient needs and changing conditions.
- Travel within the designated geographic area to reach patients’ homes.
Qualifications and Desired Experience:
- Minimum one year certified home care occupational therapy experience.
- At least two years experience in acute care, rehabilitation, or home care settings.
- Licensed Occupational Therapist registered with the Massachusetts Board of Registration.
- Proficiency with computer software related to patient care documentation; familiarity with hand-held devices is preferred.
- Strong communication skills, both verbal and written.
- Ability to prioritize tasks and adapt to a dynamic work environment.
- Comfortable traveling within the designated service area.
