Provide specialized occupational therapy services to patients across a broad age range, from infants to seniors, in their homes. This role focuses on addressing cognitive and physical impairments resulting from injury, disease, disability, or other factors. Collaborate closely with patients, their families, physicians, and other healthcare professionals to develop and coordinate personalized care plans that optimize patient outcomes. Manage diverse resources to ensure efficient and effective delivery of therapy services.

Key Responsibilities:

  • Deliver occupational therapy treatments tailored to individual patient needs within home settings
  • Assess patient conditions and collaborate with healthcare teams to coordinate multidisciplinary care
  • Utilize technical, environmental, and financial resources to support therapy goals
  • Maintain thorough documentation and utilize computer systems for patient care management
  • Adapt to a dynamic work environment requiring travel throughout the designated geographic area

Desired Qualifications:

  • Licensed Occupational Therapist registered with the appropriate state board
  • Minimum one year certified home care OT experience
  • At least two years of practice in acute care, rehabilitation, or home care settings
  • Proficiency with various computer software, with preference for experience using handheld patient care devices
  • Strong customer service aptitude and ability to serve diverse populations effectively
  • Excellent verbal, written, and electronic communication skills
  • Capacity to prioritize multiple tasks and adapt to change fluidly
  • Willingness and ability to travel within the assigned geographic region
