Schedule/Hours:

  • Two options available: Guaranteed Hours (36 hours/week with productivity minimums) or Non-Guaranteed Hours for greater flexibility.
  • Weekend coverage may be required.
  • Flexibility to travel outside the primary service area for the occasional assignment.

Qualifications:

  • Current and valid CA Occupational Therapist (OT) license.
  • Basic Life Support (BLS) certification.
  • Valid driver’s license, auto insurance, and vehicle registration.
  • Up-to-date immunizations: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), annual flu shot.
  • Drug screening and physical.
  • Home health experience preferred, but new graduates are welcome.

Responsibilities:

  • Conduct comprehensive patient evaluations, treatments, and discharge planning in home health settings.
  • Complete OASIS documentation for assessments and recertifications.
  • Develop individualized care plans and collaborate closely with care teams and families.
  • Remain flexible to meet caseload needs, including occasional weekend and cross-territory visits.
