Schedule/Hours:

• Options available for guaranteed or non-guaranteed hours
• Weekend coverage may be required
• Caseload may involve territories including Fremont, Castro Valley, Hayward, Oakland, Berkeley, San Ramon, Pleasanton, and neighboring areas
• Flexibility to travel outside assigned area to meet caseload expectations

Key Qualifications:

• Current CA Occupational Therapist license
• BLS certification
• Valid driver's license, auto insurance, and vehicle registration
• Health screenings: TB (annual), MMR, Varicella, HepB, TDap (within last 10 years), Flu (annual), drug screen, physical
• Excellent organizational, communication, and documentation skills
• Comfort with home health visits; prior home health experience preferred but not required
• Ability to see COVID+ or PUI patients as needed

Primary Responsibilities:

• Deliver comprehensive OT evaluations and personalized treatment plans in patients’ homes
• Carry out OASIS assessments (Recertification, RoC, Discharge, etc.)
• Maintain thorough visit documentation and communicate progress with physicians and interdisciplinary teams
• Travel within the assigned region, adapting to a diverse patient base and varying caseloads
