Schedule/Hours:
- Options available for guaranteed or non-guaranteed hours
- Weekend coverage may be required
- Caseload may involve territories including Fremont, Castro Valley, Hayward, Oakland, Berkeley, San Ramon, Pleasanton, and neighboring areas
- Flexibility to travel outside assigned area to meet caseload expectations
Key Qualifications:
- Current CA Occupational Therapist license
- BLS certification
- Valid driver's license, auto insurance, and vehicle registration
- Health screenings: TB (annual), MMR, Varicella, HepB, TDap (within last 10 years), Flu (annual), drug screen, physical
- Excellent organizational, communication, and documentation skills
- Comfort with home health visits; prior home health experience preferred but not required
- Ability to see COVID+ or PUI patients as needed
Primary Responsibilities:
- Deliver comprehensive OT evaluations and personalized treatment plans in patients’ homes
- Carry out OASIS assessments (Recertification, RoC, Discharge, etc.)
- Maintain thorough visit documentation and communicate progress with physicians and interdisciplinary teams
- Travel within the assigned region, adapting to a diverse patient base and varying caseloads
