Schedule/Hours:

• Options include Guaranteed hours (minimum productivity expectation) or Non-Guaranteed hours with more flexible caseloads

Key Qualifications:

• Current California Occupational Therapist License (OT)
• Basic Life Support (BLS) certification
• Valid driver's license, auto insurance, and vehicle registration
• Up-to-date immunizations (TB, MMR, Varicella, HepB, Tdap, Flu)
• Physical and drug screen clearance
• New graduates welcome; prior home health experience a plus
• Willingness to see a range of patient populations, including Covid+ and Covid PUI cases

Responsibilities:

• Provide comprehensive occupational therapy evaluations and individualized care plans in patients’ homes
• Complete OASIS and other required documentation for evaluations, visits, and discharges
• Engage with patients, families, and interdisciplinary teams to ensure best outcomes
• Travel throughout assigned territories and adapt to varying patient and facility needs
• Meet weekly productivity or caseload expectations based on hour guarantee option
