Schedule/Hours:
- Options include Guaranteed hours (minimum productivity expectation) or Non-Guaranteed hours with more flexible caseloads
- Weekend coverage may be required
- Flexibility to occasionally travel outside the primary service area
Key Qualifications:
- Current California Occupational Therapist License (OT)
- Basic Life Support (BLS) certification
- Valid driver's license, auto insurance, and vehicle registration
- Up-to-date immunizations (TB, MMR, Varicella, HepB, Tdap, Flu)
- Physical and drug screen clearance
- New graduates welcome; prior home health experience a plus
- Willingness to see a range of patient populations, including Covid+ and Covid PUI cases
Responsibilities:
- Provide comprehensive occupational therapy evaluations and individualized care plans in patients’ homes
- Complete OASIS and other required documentation for evaluations, visits, and discharges
- Engage with patients, families, and interdisciplinary teams to ensure best outcomes
- Travel throughout assigned territories and adapt to varying patient and facility needs
- Meet weekly productivity or caseload expectations based on hour guarantee option
