Provide specialized occupational therapy services to patients in their homes, addressing physical and cognitive impairments resulting from injury, disease, or disability. This role supports patients across the lifespan, from infants to the elderly, and requires collaboration with patients, families, physicians, and multidisciplinary teams to develop comprehensive care plans. Manage technical, environmental, and financial resources to optimize patient outcomes.
Key Responsibilities:
- Deliver patient-centered occupational therapy interventions in residential settings.
- Assess patient needs and coordinate with healthcare providers and family members.
- Document care plans and patient progress using electronic health records and handheld devices.
- Adapt treatment plans based on patient response and evolving clinical situations.
- Travel within the assigned geographic area to provide home care services.
Qualifications and Experience:
- Current occupational therapy licensure registered with the Massachusetts Board of Registration.
- Minimum of two years’ experience in acute care, rehabilitation, or home health settings.
- At least one year of certified home care occupational therapy experience.
- Proficient in using computer software relevant to patient care documentation; familiarity with handheld devices preferred.
- Strong communication skills, both verbal and written, to interact effectively with diverse populations.
- Ability to manage multiple priorities and work independently in changing situations.
- Willingness to travel within the designated geographic area.
