Schedule/Hours:

• Options include guaranteed 36 hours/week (with productivity requirements) or non-guaranteed hours on a per-visit basis
• Weekend coverage may be assigned, with occasional travel across counties to maintain a full caseload

Responsibilities:

• Provide patient-centered occupational therapy services in home settings
• Complete evals, follow-ups, recertifications, and discharges (OASIS and non-OASIS visits)
• Coordinate with interdisciplinary teams, patients, and physicians
• Maintain accurate documentation, travel to patient homes, and uphold productivity standards

Qualifications:

• Current Occupational Therapist license in California
• BLS certification
• Valid driver’s license, auto insurance, and reliable vehicle
• TB test (annual), MMR, Varicella, HepB, Tdap (10 years), Flu (annual)
• Ability to work with COVID+ and PUI patients
• New graduates welcome; home health experience is a plus
