Qualifications:

• Active California Occupational Therapy License (or in process under the 60-day rule)
• Previous SNF experience preferred; mentorship available for recent graduates
• Strong documentation and organizational skills
• Flexibility, collaboration, and adaptability are essential

Responsibilities:

• Conduct comprehensive evaluations and deliver effective OT interventions for SNF residents
• Collaborate with an interdisciplinary team to develop individualized care plans
• Complete documentation and records management in Net Health, maintaining facility standards
