Responsibilities:

• Evaluate and treat patients in acute care and nursing home settings
• Conduct patient screenings and actively participate in progress meetings
• Accurately document evaluations, progress notes, daily notes, insurance information, and discharge plans in medical records
• Collaborate closely with interdisciplinary team members for coordinated patient care
• Perform outpatient evaluations and treatment sessions
• Take on additional duties as assigned

Qualifications:

• Master’s degree in Occupational Therapy required; Doctorate preferred
• National Board Certification in Occupational Therapy (NBCOT)
• Active Wisconsin Occupational Therapy license and commitment to ongoing education
• Current BLS certification
• Valid driver’s license
• Minimum of one year relevant clinical experience
• Strong attention to detail and accuracy
