Schedule/Hours:
- Travel assignment with flexible scheduling options
Responsibilities:
- Evaluate and treat patients in acute care and nursing home settings
- Conduct patient screenings and actively participate in progress meetings
- Accurately document evaluations, progress notes, daily notes, insurance information, and discharge plans in medical records
- Collaborate closely with interdisciplinary team members for coordinated patient care
- Perform outpatient evaluations and treatment sessions
- Take on additional duties as assigned
Qualifications:
- Master’s degree in Occupational Therapy required; Doctorate preferred
- National Board Certification in Occupational Therapy (NBCOT)
- Active Wisconsin Occupational Therapy license and commitment to ongoing education
- Current BLS certification
- Valid driver’s license
- Minimum of one year relevant clinical experience
- Strong attention to detail and accuracy
