Embark on a rewarding assignment as a Physical Therapist delivering exceptional therapy in a hospital-affiliated home health setting. This contract opportunity offers the chance to provide skilled care to adult patients with chronic diseases and orthopedic needs across Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and neighboring communities. You’ll collaborate closely with a supportive interdisciplinary team, ensuring patients receive the highest level of care.

Schedule/Hours:

  • No weekend rotation
  • No call requirements
  • No major holidays
  • 13-week schedule cycle
  • 6 home visits per day (average 60–80 miles of travel, with weekly mileage reimbursement)

Required Qualifications:

  • Current Physical Therapy (PT) license for the state
  • Basic Life Support (BLS) certification
  • Valid driver’s license and proof of insurance
  • Minimum one year of recent home health experience
  • Will consider well-qualified first-time travelers experienced in home health

Desired Skills & Experience:

  • Comfort working independently and scheduling patient visits (must coordinate with patients the day prior to visits)
  • Familiarity with EPIC Rover (view only), Focura for documentation, Samsung Note 9 devices, and Home Care Home Base platform
  • Ability to deliver patient-centered care to adults with chronic and post-surgical orthopedic conditions

Responsibilities:

  • Provide comprehensive therapy to adult patients in their homes
  • Complete and upload relevant documentation, including discharge instructions and wound images
  • Maintain required minimum of 30 weekly visit equivalents (cancelled appointments do not count toward quota)
  • Collaborate effectively with clinical and administrative leadership
  • Uphold departmental standards and contribute to a positive team environment
