Physical Therapist - Albemarle, NC

A rewarding contract opportunity is available for an experienced Physical Therapist specializing in hospital-based home health care. This position requires providing comprehensive therapy services to adult patients with chronic diseases and those recovering from orthopedic surgery throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas. With a supportive, collaborative culture and strong departmental leadership, you’ll deliver high-quality care directly to patients in their homes.

Schedule/Hours:

• No weekend rotation
• No on-call responsibilities
• No major holiday shifts
• Scheduling cycle runs every 13 weeks
• Schedule your patient visits a day in advance

Qualifications:

• Minimum of 1 year home health experience required
• Open to seasoned first-time travelers
• Must hold an active Physical Therapist state license
• Basic Life Support (BLS) certification required
• Valid driver’s license and proof of insurance required

Responsibilities:

• Conduct six patient visits per day, averaging a total drive of 60-80 miles daily (mileage reimbursed weekly)
• Provide therapy to adults with chronic disease and after orthopedic surgery using evidence-based practices
• Schedule visits directly with patients and coordinate care to achieve 30 visit equivalents per week
• Utilize provided technology including EPIC Rover (view only), Focura, Samsung Note 9 phones, and Home Care Home Base for documentation and communication

• Strong emphasis on teamwork and collaboration within a professional environment
• Comprehensive orientation including new employee onboarding and five days of unit-specific training
• Grey scrubs required (parking provided at no cost)
