Responsibilities:

  • Complete comprehensive evaluations, create and update individualized treatment plans, and set measurable goals aimed at optimal patient rehabilitation and community reintegration.
  • Ensure physician-directed care and maintain detailed, timely medical documentation that meets clinic, federal, and state guidelines.
  • Collaborate closely with other therapy providers, delegating and directing therapy services as appropriate.
  • Serve as an educator to patients, families, peers, and students, fostering a learning-rich environment.
  • Uphold personal and team safety by practicing and promoting strict safety protocols.
  • Pursue active professional development, leveraging exposure to new workplace practices and continuous education opportunities.

Qualifications:

  • Valid PT licensure and compliance with state and regulatory requirements.
  • Previous outpatient therapy experience preferred.
  • Strong commitment to patient-first care, safety, and ethical practice.
  • Excellent communication skills and ability to work as part of a multidisciplinary team.
