An opportunity is available for a Physical Therapist with strong hospital-based experience to support home health patients for an assignment in California. This position focuses on providing skilled therapy care to a diverse patient population, including those recovering from orthopedic procedures and geriatric patients with complex medical needs. The average daily home health census is 78 and is expected to increase as new therapists join, offering a robust clinical environment for growth and impact.
Schedule:
- Day shifts
- Occasional Saturday rotation for orthopedic patients
- Holidays off
Qualifications:
- Valid Physical Therapist license in California
- Minimum 2 years of recent experience in hospital-based or home health therapy
- Active Basic Life Support (BLS) certification
- Familiarity with electronic medical records; Netsmart experience a plus
- Ability to safely perform approximately 5 daily patient visits and manage travel within the service area (average 40 miles per day, mileage continually optimized)
Responsibilities:
- Conduct initial and follow-up therapy visits, including start-of-care assessments (especially for orthopedic patients without nursing orders)
- Deliver effective, evidence-based physical therapy to promote patient recovery and prevent hospital readmission
- Collaborate with intake RN, managers, and nurse aides for integrated patient care
- Document thoroughly and accurately in the electronic medical record system
- Educate patients and families to maximize independence and improve quality of life
- Participate in care team communications to ensure efficient workload distribution and high-quality outcomes
