Take your career further by serving a diverse patient population—primarily orthopedic and geriatric clients with complex comorbidities (CHF, COPD). You’ll collaborate with experienced intake nurses, a dedicated manager, and strong support staff, deepening your understanding of varied clinical approaches as you impact patient outcomes in a close-knit home health setting.
Schedule/Hours:
- Day shifts only
- No weekends or holiday shifts required
Key Responsibilities:
- Complete Start of Care (SOC) visits, particularly orthopedic patients when nursing services are not ordered
- Deliver patient-centered physical therapy in home settings, averaging five visits per day across Tehama County (mileage generally within 40 miles daily)
- Utilize the HomeCare HomeBase (HCHB) EMR on a provided tablet to document patient care
- Work collaboratively with intake RNs, management, and nurse aides to ensure continuity of quality care
Qualifications:
- Active PT license
- BLS certification required
- At least some home health experience (minimum requirement regardless of total years in practice)
- Comfort and proficiency with diverse diagnoses, especially orthopedic and geriatric, is highly valued
- Ability to travel for work within the local region
