Key Responsibilities:

  • Conduct comprehensive patient assessments to develop individualized therapy plans
  • Deliver physical therapy treatments and interventions in patients' homes

  • Educate patients and family members on exercises and mobility techniques
  • Maintain clear, accurate documentation in compliance with regulatory standards

Qualifications and Experience:

  • A minimum of 2 years of experience in physical therapy, preferably in home health or hospital settings
  • Active Physical Therapist license in California
  • Current Basic Life Support (BLS) certification
  • Strong interpersonal and communication skills to engage effectively with patients and families
