This contract opportunity is ideal for an experienced Physical Therapist eager to deliver high-quality care in both Marian and Arroyo Grande regions. You'll be responsible for home-based care within a defined geographical area, working closely with permanent team members, including Rehab Aides, OTAs, and PTAs.
Work Schedule:
- Flexible schedule with the majority of hours Monday through Friday, 8:00 am – 5:00 pm
- Weekend rotation required every third weekend
- Minor holidays scheduled; major holidays only if needed
What You’ll Bring:
- At least two full years of hands-on professional experience
- Current, valid California PT license
- Active BLS certification
- Proficiency with Cerner EMR
- Familiarity with Oasis documentation (required)
- Background in PT/OT/Speech therapy, including experience with orthopedic, stroke, joint, and spinal injury cases
- Previous experience in the home health setting is preferred, but not required
Role Responsibilities:
- Assess, plan, and deliver individualized therapy services to patients in their homes
- Collaborate with a multidisciplinary rehab team to coordinate comprehensive care
- Maintain thorough documentation in compliance with government-mandated forms
- Attend orientation (1.5–2 weeks) to ensure confidence and familiarity with protocols
- Represent the organization professionally in appropriate business attire with a white lab coat
