Make a meaningful impact on patient well-being by providing skilled therapy services in a supportive home health setting. This contract opportunity is ideal for an experienced Physical Therapist eager to deliver high-quality care in both Marian and Arroyo Grande regions. You'll be responsible for home-based care within a defined geographical area, working closely with permanent team members, including Rehab Aides, OTAs, and PTAs.

Work Schedule:

  • Flexible schedule with the majority of hours Monday through Friday, 8:00 am – 5:00 pm
  • Weekend rotation required every third weekend
  • Minor holidays scheduled; major holidays only if needed

What You’ll Bring:

  • At least two full years of hands-on professional experience
  • Current, valid California PT license
  • Active BLS certification
  • Proficiency with Cerner EMR
  • Familiarity with Oasis documentation (required)
  • Background in PT/OT/Speech therapy, including experience with orthopedic, stroke, joint, and spinal injury cases
  • Previous experience in the home health setting is preferred, but not required

Role Responsibilities:

  • Assess, plan, and deliver individualized therapy services to patients in their homes
  • Collaborate with a multidisciplinary rehab team to coordinate comprehensive care
  • Maintain thorough documentation in compliance with government-mandated forms
  • Attend orientation (1.5–2 weeks) to ensure confidence and familiarity with protocols
  • Represent the organization professionally in appropriate business attire with a white lab coat
