Deliver exceptional patient care as a physical therapist in a supportive, team-oriented home health department. You’ll work with adult patients facing chronic diseases and orthopedic conditions, traveling to provide therapy services in communities such as Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas.

Schedule/Hours:

  • No weekend or on-call rotations
  • No major holiday requirements
  • Schedule cycle operates every 13 weeks
  • Ability to plan patient visits a day in advance
  • RTO subject to manager approval

Qualifications/Skills:

  • Current state Physical Therapy licensure (PT)
  • BLS certification required
  • Valid driver’s license and proof of insurance
  • Minimum 1 year of home health experience (seasoned first-time travelers welcome)
  • Comfortable with hospital-based home health therapy
  • Familiarity with EPIC Rover, Focura, and Home Care Home Base preferred

Responsibilities:

  • Conduct six home visits each day, driving an average of 60-80 miles daily (mileage reimbursement provided)
  • Assess, plan, and administer therapy to adult patients with chronic and orthopedic diagnoses
  • Upload discharge instructions, wound images, and other patient data using provided technology
  • Collaborate closely with a team including departmental leadership, schedulers, clinical managers, and peers
  • Maintain documentation per facility standards
