Travel Physical Therapist | Berkeley, California

Schedule / Hours:

• 36 hours per week guaranteed for eligible assignments; must reach productivity targets (27 points/week)
• Potential for weekend shifts
• Flexibility to travel outside standard service areas may be required

Qualifications:

• Current California Physical Therapy license
• Active BLS certification
• Valid driver’s license, auto insurance, and vehicle registration
• Up-to-date immunizations: TB (annual), MMR, Varicella, HepB, Tdap (within 10 years), Flu (annual)
• Must clear a drug screening and physical evaluation
• Willingness to see Covid+ and PUI patients when required

Key Responsibilities:

• Evaluate and treat patients in their homes across various East Bay territories
• Complete accurate and timely documentation (OASIS and routine home health visits)
• Coordinate care with physicians and interdisciplinary teams
• Meet productivity targets and adapt to varying caseloads and locations
• Communicate effectively with patients, families, and colleagues
