Schedule / Hours:
- 36 hours per week guaranteed for eligible assignments; must reach productivity targets (27 points/week)
- Potential for weekend shifts
- Flexibility to travel outside standard service areas may be required
Qualifications:
- Current California Physical Therapy license
- Active BLS certification
- Valid driver’s license, auto insurance, and vehicle registration
- Up-to-date immunizations: TB (annual), MMR, Varicella, HepB, Tdap (within 10 years), Flu (annual)
- Must clear a drug screening and physical evaluation
- Willingness to see Covid+ and PUI patients when required
Key Responsibilities:
- Evaluate and treat patients in their homes across various East Bay territories
- Complete accurate and timely documentation (OASIS and routine home health visits)
- Coordinate care with physicians and interdisciplinary teams
- Meet productivity targets and adapt to varying caseloads and locations
- Communicate effectively with patients, families, and colleagues
