Schedule/Hours:
- 36-hour minimum weekly guarantee applies for qualifying travelers, with at least 27 productivity points expected per week.
- Potential for weekend coverage as needed.
- Flexibility to travel outside the primary service area to secure a full caseload and fulfill visit requirements.
Required Qualifications:
- Current and valid California Physical Therapist license.
- American Heart Association Basic Life Support (BLS) certification.
- Valid driver’s license, up-to-date auto insurance, and auto registration.
- Recent (annual) TB test; immunizations including MMR, Varicella, Hepatitis B, Tdap (within 10 years), and annual flu shot.
- Successful completion of drug screening and physical examination.
- Willingness to travel between patient locations in specified counties with occasional assignments in alternate service areas.
- Previous experience in hospital or home health therapy preferred.
Key Responsibilities:
- Provide comprehensive physical therapy evaluations and implement tailored treatment plans for patients in hospital and home settings.
- Conduct visits and documentation according to established points system (OASIS assessments, routine visits, recertifications).
- Manage an assigned territory, occasionally traveling beyond to maintain visit expectations.
- Deliver care to patients of varying acuity, including Covid+ and PUI cases as needed.
- Complete all clinical documentation, patient communications, and timely reporting.
- Collaborate with interdisciplinary teams and maintain effective communication with physicians and caregivers.
