Physical Therapist | Chandler, Arizona

Provide expert physical therapy evaluation and treatment services in an outpatient setting, focusing on individualized, functional care plans that support patients’ return to community activities. This role includes delegation and coordination of discipline-specific therapy services, patient and family education, and contributing to the development and delivery of therapy programs that align with clinical goals and physician directives.

Key Responsibilities:

• Conduct comprehensive patient evaluations and develop tailored treatment plans focused on patient goals and discharge readiness.
• Document all aspects of care clearly, accurately, and timely, including assessments, goals, letters of medical necessity, and charge entry in compliance with regulatory and organizational guidelines.
• Oversee and direct therapy services for assigned patients, ensuring care aligns with valid physician orders and insurance requirements.
• Educate patients, families, students, and colleagues to promote understanding and effective treatment outcomes.
• Participate in student clinical education by providing supervision, setting learning objectives, and completing evaluations.
• Maintain professional development through continuing education, self-study, and active involvement with professional organizations.
• Adhere to state practice acts, ethical standards, and safety protocols, including patient and employee safety initiatives.
• Support organizational performance improvement initiatives through active participation and feedback.

Qualifications & Desired Experience:

• Valid physical therapy licensure in good standing.
