Deliver essential therapy services within a dynamic home health setting focused on adult patients with chronic diseases and orthopedic conditions. This contract role provides an opportunity to work with a dedicated clinical team, utilizing modern technology and supportive departmental leadership. Candidates will travel across multiple areas, ensuring comprehensive care and patient engagement.

Schedule:

  • No weekend rotation
  • No on-call requirements
  • No major holidays scheduled
  • Schedule cycle: every 13 weeks

Qualifications:

  • Active state Physical Therapist license (PT)
  • Valid driver’s license and proof of insurance required
  • Current BLS certification
  • Minimum of 1 year of recent home health experience
  • Open to candidates new to travel, if seasoned in home health
  • Strong ability to self-manage, travel daily, and adapt to a varying caseload

Responsibilities:

  • Complete an average of 6 home therapy visits per day across a broad service area (Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding communities)
  • Independently schedule next-day patient appointments
  • Deliver evidence-based physical therapy assessments and interventions
  • Document patient care accurately using Home Care Home Base, Samsung Note 9, and Focura platforms; limited view-only access to EPIC Rover
  • Communicate efficiently with patients, providers, and a supportive team including schedulers, clinical managers, and supervisors
  • Participate actively in a collaborative, team-oriented department culture
