Deliver vital patient care as part of a dedicated home health therapy team serving adult populations with chronic diseases and post-orthopedic surgery needs. This contract assignment is ideal for physical therapists with at least one year of home health experience and a drive to provide exemplary care across a variety of community settings. Flexibility and strong organizational skills are essential, as you’ll manage your own daily schedule of six home visits, traveling an average of 60 to 80 miles per day throughout the service area. Mileage is reimbursed weekly.

Schedule/Hours:

  • No weekend rotation
  • No on-call duties
  • No major holidays required
  • Schedule operates on a 13-week cycle

Qualifications:

  • Active state licensure as a Physical Therapist (PT)
  • Current BLS certification
  • Valid driver’s license and proof of insurance
  • At least one year of home health experience is required; first-time travelers considered if experienced in home health settings

Key Duties:

  • Conduct home visits with adult patients, managing a caseload to achieve a minimum of 30 visit equivalents weekly
  • Assess, plan, and implement therapy interventions for chronic disease and orthopedic recovery
  • Coordinate daily appointments directly with patients
  • Accurately document all visits using Home Care Home Base, Focura, and provided Samsung Note 9 devices
  • Collaborate with a supportive interdisciplinary team that includes leadership, schedulers, clinical managers, supervisors, and peers
