A reputable hospital in California is seeking an experienced Physical Therapist for a contract opportunity focused on Home Health services. This dynamic role primarily supports orthopedic patients following joint replacement procedures, along with geriatric clients with complex medical needs such as CHF and COPD. The successful candidate will become an integral part of a growing therapy team, contributing to a patient-centered approach that maximizes independence and quality of life for individuals recovering at home.

Schedule/Hours:

  • Day shifts
  • Occasional Saturday coverage for orthopedic patients
  • Holidays off

Key Responsibilities:

  • Conduct Start of Care (SOC) visits, especially for orthopedic cases without concurrent nursing orders
  • Manage a caseload of approximately five patient visits per day within the diverse Tehama County service area
  • Maintain accurate clinical notes using Netsmart EMR (laptop provided)
  • Collaborate with an interdisciplinary team, including RN Intake and management support
  • Promote patient safety and restorative outcomes in the home setting

Desired Qualifications:

  • Minimum of two years’ physical therapy experience, with an emphasis on home health or acute care
  • Active California Physical Therapist license in good standing
  • Current BLS certification
  • Strong knowledge in managing orthopedic and geriatric conditions
  • Comfortable with daily travel (average 40 miles) within the regional service area, with mileage expected to decrease as caseload is redistributed
  • Exceptional clinical, assessment, and communication skills
